Extract the archive to your foobar2000 folder (default is C:\Program Files\foobar2000). Launch foobar2000 and make sure the interface module is Columns UI (File > Preferences or Ctrl+P). In the preferences window go to Display: User interface module. If it isn't, select it and restart foobar2000.
Next, go to Preferences > Display > Columns UI > Main and select FCL importing and exporting: Import...
Open the file: layout\simplicity.fcl
You may want to disable the toolbars and the status bar, you can do that on the same screen (Misclleaneous: Show toolbars) and on the Status bar tab (Status bar: Show status bar)

Playlist gets the artwork from Columns UI artreader. If that's not available, it tries to reach the defined artwork by looking for folder.jpg or folder.png in the track directory.
Clicking on rating stars writes the rating into the file tag (RATING).
